Immunization Requirements

Mount Vernon City Schools recently sent an email informing you about the State of Ohio's immunization requirements for 7th and 12th-grade students. If your child has not yet received the required vaccines or if MVCSD has not received the immunization exempt form, please get in touch with your student's school.  
  •  7th-grade students are required to have one dose of the Tdap and Meningococcal (serogroup A, C, W, and Y) vaccine
  •  12th-grade  students are required to have a second dose of the Meningococcal (serogroup A, C, W, and Y) vaccine
If your child has not received the necessary immunizations or if the Immunization Exemption form has not been returned to us by Friday, October 27th, we regret to inform you that your child will NOT be allowed to attend school until these requirements are met. 

If you choose not to vaccinate your child, an Immunization Exemption Form must be completed. Depending upon your browser, you may have to download the document to enable fillable fields. Return the completed form to your child’s school secretary or send it as an attachment to [email protected].

We still have some appointment times for Thursday in our  School-Based Health Center, operated by Nationwide Children's Hospital located in the back of the high School. Immunization Clinic Thursday, October 26th, from 8:00 AM to 4:00 PM. 

  • Please select the Universal Consent Form link. You will have to download and fill out the document.
  • Schedule an appointment with the School-Based Health Center by selecting the NCH Appointment link and choosing an appointment time slot.
  • Bring your completed Universal Consent form and a copy of your insurance card to the appointment.
