Kindergarten Enrollment

STEP 1 - CREATE a new “PowerSchool” parent account. 

Once you select  Create Account, you will be asked to fill out a brief form including personal information and account access information.
When finished, select the Create Account button.

Step 2: COMPLETE the online registration. 

Once you have created your Parent Portal account, you may begin your child’s enrollment.  You will be asked to complete nine sections. All sections must be completed before enrollment can be processed.

You may select Save & Sign Out at any time (located in the top right corner under your initials) to save your progress and return later.Select Your Enrollment Type

Open Enrollment - you must choose one of the following options:

• Open Enrollment - outside of Mount Vernon City School District
  • Students who live outside the Mount Vernon City School District may apply to attend a Mount Vernon City School. Acceptance is based on district policy and available space.
• Open Enrollment - within Mount Vernon City School District
  • Students who live within the Mount Vernon City School District may request to attend a different elementary school instead of their assigned home school. Approval is based on space availability.

Select the grade your child will be entering on their first day of school in the Mount Vernon City School District.

3. Transportation - indicate whether bus transportation is required (note: transportation is not provided for open enrollment students).
4. Custody - indicate the student’s legal guardian. In Ohio, a legal guardian may be a biological or adoptive parent, or an individual appointed by a court who has been granted legal responsibility for the child. 
5. Contacts - list only the student’s biological parent(s), adoptive parent(s), court-appointed legal guardian(s), or court-designated legal custodian(s). Do not include individuals who do not have legal custodial rights.
6. Emergency Contacts - provide the names and phone numbers of trusted adults the school may contact in the event of an emergency if a parent or guardian cannot be reached. Please indicate whether each emergency contact is authorized to pick up the student from school. Do not repeat any parent or guardian already listed in the contacts section.
7. Immunization History - upload the student’s immunization history.
8. Medical - please include only medical conditions that have been formally diagnosed by a licensed medical professional.
9. Prescriptions - If medication is to be administered at school, the required medication authorization form must be completed by the student’s physician. The parent or guardian, only, must submit the completed form to the school office along with the medication. 
10. Agreements -  The parent/guardian and the student must review and formally acknowledge the school or district’s policies before enrollment is completed.
11. Documents - This section is where you will upload required documents, including immunization records, the student's health record (a form will be provided for you to download and complete), a parent/guardian state-issued ID, the student’s birth certificate, and proof of residency.
12. Signature

Summary  The Summary page will notify you of any missing information. A red circled number shows how many items need attention, and a green check mark means the section is complete. Click a section name to return and finish any remaining items.

• When you have finished all sections, click the Submit button at the top or bottom of the form to securely send your student’s information to the district.
