To register for the 2026–2027 school year, youngsters must be five years old on or before the first day of kindergarten instruction, which is on August 19, 2026.

In Ohio, a student may be enrolled in school by a biological or adoptive parent, a legal guardian appointed by the court, or an individual who has been granted legal custody through a court order. In certain circumstances, a grandparent or other caregiver may enroll a child if they have proper legal documentation (such as a power of attorney or caretaker authorization affidavit) in accordance with Ohio law.

MVCSD requires official documentation to verify legal authority before completing enrollment.

Proof of Residency - one of the following

• Mortgage statement
• Utility bill (gas, electric, or water)
• Lease agreement
• Deed
• Property tax statement
• Valid Photo ID with current address

Child’s Birth Certificate
An official birth certificate or certified copy.

Immunization Records
Up-to-date vaccination records in accordance with Ohio Department of Health requirements.

Parent/Guardian Identification
Valid photo ID of the enrolling parent or legal guardian.

Custody Documentation (if applicable)
Current Documentation of Custody (if applicable).  In accordance with Ohio Revised Code 3313.67.2 and the Missing Children’s Act, the parent/guardian must provide a certified copy of the divorce decree and parenting plan allocating parental rights and responsibilities and/or other documents allocating custody or guardianship.

Health Information
If medication is to be administered at school, the required medication authorization form must be completed by the student’s physician. The parent or guardian must submit the completed form to the school office along with the medication.

During the enrollment process, you will be asked to provide information regarding your child’s relevant medical history, diagnosed medical conditions, current medications, allergies, and any existing health care plans. Please include only medical conditions that have been formally diagnosed by a licensed medical professional.

Previous School Records (if applicable)
Report cards, transcripts, withdrawal forms, IEP/504 plans, or other academic records.
