2023-2024 Online Forms
Reminder post-it note.
MVCSD wants to remind you to log in to your OneView Parent Portal account and fill out the online forms for your student(s).

Each year, all parents are required to submit two forms, with the Free and Reduced Lunch Form also available.
  • An updated Emergency Medical Form and
  • District Authorizations Form
  • Free and Reduced Lunch Form - The Free and Reduced lunch forms are confidential, and we encourage all families to complete the free-reduced lunch and breakfast applications.  New financial criteria have been established, and we believe that families who qualify for 'reduced' status may now be eligible to receive free lunch and breakfast under the most recently passed biennial budget. When the Free or Reduced application is approved, it will impact lunch and school fees.
Middle and High School Athletes will have an additional Athletics Form that must be submitted.

We request that you check the contact information in your student’s emergency medical form. Update any email, cell phone numbers, and address changes since last year.  The emergency medical form contact information is the contact data imported into our communication system.

If you are new to the district and have completed the enrollment form, you will still need to log back into your OneView Parent Portal review the Emergency Medical Form, complete the short District Authorizations Form, and the Free/Reduced Lunch Form.

Guidelines and instructions for completing the necessary forms are linked on the Parents Resource page.
On the District Website, select Parent Resources under the FOR FAMILIES menu for directions on creating an account and submitting online forms. If you are familiar with OneView, log in to the Mount Vernon OneView Portal and fill out your student’s forms.
Mount Vernon City Schools, will continue to use electronic form submissions and student management systems through our Mount Vernon OneView. The OneView Parent Portal will be the main access site for all school forms including athletics, bus routes, ProgressBook access, student fees, and lunches.

Call your building secretary for assistance if you cannot access a computer. You may also stop by the Board of Education Office at 300 Martinsburg Road; a computer will be provided. For parents to create accounts in OneView or complete online forms, computers will be available at individual buildings.

District parents will receive an email reminder with directions on accessing the Parent Portal login and completing the required student forms for the 2023-24 school year.

School bus route assignments and details will be available in mid-August.  Keep checking back periodically; as new students enroll, routes and times are subject to change.
